Assessment
Case Nurse will meet the client and carer on site for care plan assessment.
Planning
Case Nurse will discuss the most suitable care plan with the client and family.
Execution
Once the care plan is confirmed, it will be executed on agreed schedule by the care team with ongoing monitoring by the Case Nurse.
